大学教育病院における多剤耐性グラム陰性菌とメチシリン耐性黄色ブドウ球菌に関連する経済的負担の比較
Economic burden associated with multi-resistant Gram-negative organisms compared with that for methicillin-resistant Staphylococcus aureus in a university teaching hospital

本研究の目的は、オーストリアの出来高払い（performance related）の病院資金制度（LKF）を利用して、多剤耐性グラム陰性桿菌（MR-GNB）とメチシリン耐性黄色ブドウ球菌（MRSA）の感染患者の入院費用を比較検討することである。研究は、2,160床の大学教育病院であるVienna General Hospitalにおいて、2002年1月から6月に後向きに実施した。患者はMR-GNB群99例（年齢中央値58歳）、MRSA群74例（年齢中央値60歳）であった。MR-GNB群（59例、60％）は、MRSA群（25例、34％）より集中治療室で治療を受けた患者が多かった（P＜0.01）。2群における入院期間中央値（MR-GNB群42日、MRSA群37日）と死亡数（それぞれ18例、9例）は同等であった。MR-GNB群患者の総入院費用［2,605,327ポンド（中央値18,115ポンド）≒4,915,712 LKF credit point（中央値34,180）］は、MRSA群患者［1,093,906ポンド（中央値6,624ポンド）≒2,088,904 LKF credit point（中央値12,650）］より高額であった（P＜0.01）。本研究は後向きであり、また請求に基づく費用計算であるという限界がある。しかし本研究は、MR-GNB感染患者の入院費用は多額で、おそらくMRSA感染患者よりも高額であることを示唆している。
