インドネシアの病院における医療関連感染のサーベイランス ★
Surveillance of healthcare-associated infections in Indonesian hospitals

インドネシアの2カ所の教育病院(病院A、病院B)において、医療関連感染およびリスク因子への曝露に関する横断的サーベイランスを実施した。内科、外科、産婦人科、小児科などの診療科部門、および集中治療室の患者を対象とした。患者の人口統計学的特性、抗生物質の使用、培養検査結果、医療関連感染(静脈炎、手術部位感染、尿路感染、敗血症)の発現、およびリスク因子を記録した。観察者によるばらつきを調べるため、検証研究を病院Bで実施した。病院A、病院Bからそれぞれ1,334例、888例の患者がサーベイされた。侵襲性器具の使用または手術の実施は59%であった。病院Aでは、全患者のうち2.8%で静脈炎、1.7%で手術部位感染、0.9%で尿路感染、0.8%で敗血症を認めた。病院Bでは、3.8%で静脈炎、1.8%で手術部位感染、1.1%で尿路感染、0.8%で敗血症を認めた。検証研究では、第1チームが記録した罹患率は静脈炎2.6%、手術部位感染1.8%、尿路感染0.9%、敗血症0であった。第2チームが記録した罹患率は静脈炎2.2%、手術部位感染2.6%、尿路感染3.5%、敗血症0.9%であった。本研究はインドネシアで初めての医療関連感染に関する報告である。罹患率は他国のものと同等である。検証研究では罹患率に大きな差が認められたため、サーベイランスの信頼性は不十分である。サーベイランスに用いた方法は、医療関連感染レベルを推定して感染予防策を改善するための医療資源が限られている国々の病院においても実行可能なものである。サーベイランスの対象を侵襲的治療を受ける患者に限定することによって、有効性を向上させることができる。これにより、わずか60%の患者をスクリーニングすることで、すべての感染の90%を検出することができる。
