Surveillance of hospital-acquired infections based on electronic hospital registries

外科および内科病棟の入院患者の病院感染症に関する診療録レビューにより、コンピュータを利用した病院感染症サーベイランス方法を、従来の手作業による記録方法（著者らの標準的方法、すなわち標準法）と比較した。様々な電子化病院記録の中から選択した感染症パラメータを組み合わせることによって、コンピュータは94％の感度および47％の特異度で一般的な病院感染症を検出した。しかし、簡易化した基準（感染パラメータ）の組み合わせを用いて敗血症、尿路感染症、肺炎、術後創部感染症を定義したところ、コンピュータを利用したサーベイランスでは、従来のマニュアルの記録方法と比較して、これらの感染を感度は82％（尿路感染症）から100％（敗血症）の範囲、特異度は91％（術後創部感染症）から100％（敗血症）の範囲で検出できた。結論として、電子化病院記録における他の目的で収集したデータに基づくコンピュータサーベイランスは、院内感染の監視の効果的な方法である。

診療録の電子化が進む中で参考になる報告であるが、現実的にはそれぞれの病院で採用されている電子化システムでの検討も必要である。
