Impact of deep infection after hip fracture surgery on function and mortality

本研究の目的は、大腿骨頸部骨折手術後の深部創傷感染が機能的な予後および死亡率に及ぼす影響を評価することである。50歳以上（平均78.3歳）で、非病的骨折により手術を受けた連続2,276例を前向きに追跡した。深部感染が認められ（1.3％）、股関節再置換術を受けた29例を、年齢、性別、骨折時の居住形態、骨折型、治療方法、および歩行能力が一致した、感染のない対照と比較した。初回手術の4カ月後に大腿骨頸部骨折患者の機能を評価したところ、深部創傷感染患者は、対照患者と比べて歩行能力に障害があり（P＝0.039）、車椅子などの歩行の補助をより多く必要とした（48％対20.8％、P＝0.022）。4カ月時点での最初の病院での平均入院期間は、症例患者が対照患者より有意に長かった（P＜0.001）。深部感染患者では糖尿病が多く認められた（P＝0.038）。最も多く分離された微生物は黄色ブドウ球菌で、他の細菌（14.3％）と比較すると、高い1年死亡率との関連が認められた（57％、P＝0.014）。1年後の全死亡率は感染患者34.5％、対照患者24.1％であった（P＝0.508）。結論として、大腿骨頸部骨折手術後の深部感染は、短期の生活機能の転帰を悪化させるとともに死亡率をわずかに増加させ、超過死亡率は10.4％であった。
