Intermittent point prevalence surveys on healthcare-associated infections, 2011 and 2016, in England: what are the surveillance and intervention priorities?

背景

点有病率調査は、医療関連感染症（HCAI）の負担を明らかにする重要なサーベイランス法である。

目的

英国における 2 回の点有病率調査（2011 年と 2016 年）の重要な結果の概要を示すこと。

方法

英国のすべての国民保健サービスおよび独立セクターの病院が、対象として適格であった。欧州疾病予防管理センターにより立案されたプロトコールおよびコードブックに基づき、2011 年、2016 年の両年の 9 月から 11 月までのデータを収集した。Stata Version 13 と SAS Version 9.3 を用いて分析を実施した。混合効果モデルを適用して、それにより組織別の平均値の推定が可能となり、さまざまな組織からの回答の異質性を明らかにした。

結果

計 100,755 件の症例記録が含まれた（2011 年 52,433 件、2016 年 48,312 件）。HCAI の推定有病率は、2016 年（6.89％、95％信頼区間［CI］6.21％ ～ 7.57％）では、2011 年（6.41％、95％CI 5.75％ ～ 7.06％）よりもわずかに高かった。両調査とも、HCAI 有病率は成人集中治療室でもっとも高く（2011 年 23.1％、2016 年 21.2％）、HCAI の原因は肺炎／下気道感染症がもっとも多かった（2011 年 22.7％、2016 年 29.2％）。2016 年は、2011 年と比較して、急性期病院の入院患者が高齢で、死亡リスクが高かったが、HCAI 発症および抗菌薬投与中の入院患者のいずれの割合にも有意差はなかった。

結論

英国の病院の HCAI による負担は、2011 年から 2016 年までにわずかに増加した。しかし、 HCAI 発症および抗菌薬投与中の入院患者のいずれの割合にも有意差は認められなかった。

監訳者コメント：

結論にあるように、HCAI や抗菌薬投与の割合は 2011 年から 2016 年まで大きな変化はなかったが、対象となる患者の年齢は上昇し、デバイス使用率も増加した。つまり感染対策が適切に行われた可能性がある。日本でも都道府県や国レベルで、このような PPS が行われると良いだろう。
