Comparing the efficacy of povidone-iodine and normal saline in incisional wound irrigation to prevent superficial surgical site infection: a randomized clinical trial in gastric surgery

背景

胃切除術後の手術部位感染症（SSI）の予防にますます注目が集まっている。SSI を減少させるために予防的な切開創の洗浄が提唱されているが、溶液の選択については依然として議論されている。

目的

胃切除術後の SSI 予防のための生理食塩水による創洗浄とポビドンヨードによる創洗浄の有効性を比較すること、ならびに SSI のリスク因子を特定すること。

方法

本無作為化単施設臨床試験は胃癌患者 340 例を対象とした。患者を創閉鎖前の切開創洗浄のために 0.9％生理食塩水を用いる群と、1.0％ポビドンヨード溶液を用いる群のいずれかに、1：1 の割合で 2 群に無作為に割り付けた。主要エンドポイントは胃切除術の 30 日以内の術後 SSI とし、副次的エンドポイントは入院期間とした。

結果

計 333 例の患者が修正 intent-to-treat 群の対象となり、SSI の発生率はポビドンヨード群（11 例/167 例、6.59％）と生理食塩水群（9 例/166 例、5.42％）の間で有意差は認められなかった（オッズ比［OR］1.131、95％信頼区間［CI］0.459 ～ 3.712、P ＝ 0.655）。さらに、入院期間に関して、2 群間の差は有意ではなかった（P ＝ 0.301）。体格指数（BMI）（OR 2.639、95％CI 1.040 ～ 6.694、P ＝ 0.041）および術後合併症（OR 2.565、95％CI 1.023 ～ 6.431、P ＝ 0.045）が SSI の独立したリスク因子として特定された。

結論

生理食塩水とポビドンヨードは、胃切除術後の SSI 予防のための予防的な創洗浄剤として同程度の有効性を示した。BMI 値が高い患者または術後合併症を有する患者では、SSI のリスクがより高かった。

監訳者コメント：

本論文では胃がん手術の創部の洗浄液として、生理食塩水とポビドンヨードを比較したもので、両群間に差はなかった。胃がん手術における創部の洗浄液の種類についてもガイドライン毎で異なり、確実なエビデンスがないため結論に至っていないのが現状である。また、胃がん手術における SSI のリスク因子は、本論文の肥満以外に、75 歳以上の高齢もリスクとなっている。
