小児救急部における RS ウイルスの迅速検査:感染制御および病床管理での有用性

RS ウイルスは、温帯地域で毎年冬季に発生する小児の気道感染症のアウトブレイクの原因となっており、病床管理に大きな負担をもたらしている。RS ウイルスの院内伝播の減少を図るうえで、患児のコホーティングは効果的な戦略であることが示されており、また、これにより個室を必要とする患児のために個室を確保することが可能となる。症状を呈する小児の RS ウイルス検査は標準的な処置であるが、残念ながら従来の臨床検査法は迅速性に劣るため、治療法の決定プロセスに有用ではない。これに代わるものとして、救急部門での迅速ポイントオブケア検査が提唱されている。著者らは前向き研究を実施し、対象を限定したコホーティング戦略の実施を目的として、ポイントオブケア検査の結果を用いることにより確保できた個室の時間を推計した。4 か月の研究期間中に、ポイントオブケア検査を実施することによって指定のコホーティングエリアに小児 183 例が直接入院することができ、他の患児のために確保できた個室の時間は 568.5 個室・日であった。これは、研究期間中に 1 日あたり 5 室の個室を確保できたことに相当する。本稿はポイントオブケア検査の有用性を初めて定量的に評価したものである。RS ウイルスのポイントオブケア検査は、病床管理を改善するための安全で費用対効果に優れた効率的な手法である。

監訳者コメント
ポイントオブケア検査(POCT)とは、一般的には迅速診断検査のことを指す。RS ウイルスによる呼吸器感染症は広く乳幼児が罹患することから、我が国の免疫クロマトグラフ法検査の適応対象は年齢や基礎疾患により限定されている。
